A personalized care program that comes to you
Throughout our lives, each of us will need some type of medical care and
help. Some of us may need surgery or physical therapy. We may have ongoing medical
conditions that become more complicated over time. And, some of us will need to tap
into various social services as we age.
Providence Elder at Home is designed to help medically fragile seniors get the care they
need while remaining safely in the comfort of their own homes for as long as possible.
Providence Elder at Home offers the complete, well-organized care we would want for our
loved ones. It’s an innovative way to meet the health care needs of older adults.
Get the care you need – at home
Providence Elder at Home serves select Providence Health Plan Medicare Advantage
members in Clackamas, Multnomah, Washington and parts of Yamhill counties. It is
designed for people who have ongoing medical needs but have difficulty getting to doctor’s
appointments. If this describes you or a loved one, Elder at Home will bring customized care
and services to your home wherever that is.
When you choose Elder at Home, we’ll begin by talking with you and your primary care provider.
Working together, we’ll create a complete plan for your care, based on your personal health
care goals and your needs.
Elder at Home also gives you round-the-clock access to in-home medical care. If you have a
concern or need medical attention, just call us. A team member will come to your home within
60 minutes, if needed. You can avoid the stress of making a trip to the emergency department.
You’ll have peace of mind when you have immediate access to a medical team who
knows you and is familiar with your care plan.
Check with your primary care provider to determine if Providence Elder at Home is right for you or your loved one. All referrals to Elder at Home are made by primary care providers and all eligible patients must be members of a Providence Health Plan Medicare Advantage plan, aged 55+ and meet specific criteria related to chronic health conditions.
About our team
All Elder at Home team members are experienced in caring for the elderly. They may include:
- Nurse practitioners
- Physician assistants
- Registered nurses
- Licensed clinical social workers
We can also add others to the team as needed, including:
- Clinical pharmacists
- Rehabilitation specialists
These specialists will work with your own primary care provider. Together they will
monitor your health, your treatment plan, your medications, any safety concerns and your
need for support services.
Providence Elder at Home serves patients with all types of conditions and medical
needs. We respect the relationship you have with your primary care provider, and we work
closely with that provider to ensure you have complete care available from the comfort of
Elder at Home is committed to safeguarding your dignity by offering a holistic approach
