2013 Culture of Safety Survey: Your opinion needed

This month you may be asked to complete Providence’s Culture of Safety Survey. This confidential safety and attitudes questionnaire will gather thoughts from caregivers, employees and physicians in acute care, ambulatory services and Providence Home and Community Services.

As with any survey, success is measured by your honest feedback and your full involvement. We aim for 100 percent participation.

Not everyone in our region is taking the survey. Work settings have been chosen based on our goal to learn about front-line caregivers’ perceptions about teamwork, safety, job satisfaction, stress, local management, hospital leadership, and handoffs and transition.

If you receive an email from our survey vendor, Pascal Metrics, we hope you’ll take 15 minutes to speak out for safety on behalf of those we serve. You’ll have until Nov. 4 to complete the survey.
