Providence Anticoagulation and Pharmacotherapy Services work to provide you with connected care and improved health outcomes. For over 20 years, clinical pharmacists in our anticoagulation clinics have offered our patients safe, effective management of blood-thinning drugs. We have expanded to include pharmacotherapy services to support our patients, now available to manage multiple sclerosis and hepatitis C.

Medication management

Addition of a clinical pharmacy specialist to your care team provides specialized expertise to help you coordinate and effectively manage your medications. This service is especially beneficial for people with complex chronic diseases.
