About Our Survey

The Patient Rating score is based on responses given during the CAHPS Patient Experience Survey. Responses are measured on a 10-point scale, with 10 being the best score. These scores are then translated to a 5-point scale in order to display results in a 5-star rating. Comments are also gathered from the same survey and displayed in their entirety with the exception of any language that may be considered slander, libel or contain private health information, which will be removed prior to publishing the comments.

[...] Many times I get questions answered or prescription refills using email, as needed. Usually she writes a note with test results, which are timely. Also, the lab person is 5 star, as I have small veins and she gets it first attempt.

I first saw Doctor K last year in Urgent Care after a surgery, and I knew I wanted her as my primary. That is the most friendly and helpful staff I have ever dealt with.

doctor Kolonic is a wonderful provider! She is a great listener and I feel like when she comes into the exam room nothing else matters to her about what I am there for and what my needs are.she takes care of everything I've come in there to have addressed before I leave.

The doctor didn't even ask about my symptoms/concerns and just ordered tests. [...] I tried to talk to her, and she was in a hurry and quickly finished and left the room. I got maybe 5 minutes with her. [...] My experience with her nurse was much better.

I learned a lot about my bone density test--Dr. Kolonic explained my results in very plain language that was very easy to understand---at least I now know why my back hurts so much. Changing to Dr. Kolonic was the smartest and best decision I've made--at least I am getting things accomplished that should have been done a long time ago.

This was my first time visiting this office and provider. I was scheduled for a new patient appointment and from the moment I walked in the door I was disappointed. I witnessed the two front office staff complaining about every caller and gossiping. The Medical Assistant was very forgetful and disorganized. Dr. Kolonic spent 15 minutes with me for a new patient appointment, which I felt was not enough time scheduled for new patients. [...] The entire experience was very disappointing [...]

[...] I was so disappointed in this visit with Dr Kolonic and felt she was simply too busy. There were questions I wanted to ask that I didnt even get a chance to ask! I am new to this area and being so I tend to research the Drs. She had referred to another Dr who when I looked him up had horrible reviews. I wouldn't care so much if I was suffering from a cold or something minor but my issues are serious enough to scare me, so I expect top notch service from the attending physician. I will still recommend the office and certainly Dr [...] and I will make my appts with her whenever possible. [...]

Dr. Kolonic, and everyone at the clinic, make me feel like I am important as a person. I am not in an assembly line. She takes the time to talk and listen to what I have to say. On one occasion, I had brought someone else into the facility, and she took a moment to ask me how I was.

It would be nice to have my blood work done the week before my visit for a physical so that the doctor could personally review my results with me. I received an email with results, but I didn't understand what all of the tests were and didn't have a chance to ask questions.

upon entering the facility main lobby one entry clerk was working with another patient, another reception clerk (male) was typing on terminal, saw me standing and waiting. He never said a word, completely ignored me even being there. I waited for several minutes with no response from him so I filled out the self check-in form he still ignored me until I said something to him. Not good people skills, should not be first line of contact to represent your Hospital.
