About Our Survey

The Patient Rating score is based on responses given during the CAHPS Patient Experience Survey. Responses are measured on a 10-point scale, with 10 being the best score. These scores are then translated to a 5-point scale in order to display results in a 5-star rating. Comments are also gathered from the same survey and displayed in their entirety with the exception of any language that may be considered slander, libel or contain private health information, which will be removed prior to publishing the comments.

This was a "get aquainted" appt. since I am being forced to find a new provider since mine is retiring in June. If it were not for that I would not have gone here to Dr. Diaz, he has a bit of a heavy accent which is at times hard to understand and I had to have him repeat what he said several times

Dr. Diaz listened to my health issue and I was able to see a physical therapist that same day through his referral. Got bloodwork results the next day from the physical with comments from the doctor.

Doctor Diaz is a nice guy, obviously fairly knowledgeable enough about the subject matter I asked him about. A couple things though -- 1) Although he was able to have his assistants to the blood test right away, he never gave any indication of how long I'd have to wait to get the results. [...] 2) I believe that we decided to do a certain blood test to determine if I was currently infected, but infact, it appears that 2 tests were ordered [...] I have to stress that other than these two things, I was satisfied with my care with Dr Diaz.

MY PCP is amazing. His staff are terrible. [...] The ONLY reason I continue to receive treatment here is because of my physician.

[...] Dr.Diaz was a very pleasant and positive solution to my search for a primary care physician. I hope I will be able to keep him down the road. At my age, being shuffled from doctor to doctor is extremely disruptive, time-consuming and stressful.
