About Our Survey

The Patient Rating score is based on responses given during the CAHPS Patient Experience Survey. Responses are measured on a 10-point scale, with 10 being the best score. These scores are then translated to a 5-point scale in order to display results in a 5-star rating. Comments are also gathered from the same survey and displayed in their entirety with the exception of any language that may be considered slander, libel or contain private health information, which will be removed prior to publishing the comments.

Dr Diaz became my care provider when Dr [...] left the clinic. Dr Diaz is excellent and I am very satisfied with his care and would recommend him to others. He is very knowledgeable about medications, pathology and current trends. He is an asset to Providence.

I filled out a mental/emotional health survey before seeing the doctor that should have seriously raised some eyebrows, but the doctor didn't talk to me about it at all. Luckily I'm in treatment for mental health issues elsewhere, but the doctor doesn't know that. The doctor also didn't do a good job explaining the tetanus booster I was about to get, or potential side effects. I received the results of my blood test via MyCharts with no explanation or interpretation of the technical results. Generally I feel pretty poorly considered.

I felt like my doctor wasn't interested in me at all. I felt like he thinks I'm going to abuse my medication. I had some questions about dosage and frequency because my previous provider gave me two different answers and I don't feel my question was answered, but rather a reason to reduce my prescription. This was my first experience with providence and possibly my last. The prescription I ordered still han't arrived to my pharmacy and its been 7 days.

I was a bit disappointed not to have questions about ongoing blood sugar issues, or to be asked to take a blood test.

I had an MRI last Wednesday and it is now Monday & I still do not know the results. I have called the office and still nothing and no update or results have been put on My Charts. I'm disappointed that I have not been contacted yet with my MRI results.

I went for a physical exam but there was very little examination or testing. They never even had me remove any clothes. They asked some questions and drew some blood and that was pretty much it. If I didn't bring something up, it wasn't discussed. Maybe this is the new way of doing things, but I don't find it very comprehensive or thorough.

There was some miscommunication between the nurse and the doctor. They apparently think I have a prescription for the medication I have been taking, even though I clearly stated to the nurse that I have no prescription.

Para-medical assistant who took my weight & guided me into examination room, asked several general medication questions , had 'difficulty' with blood pressure measuring devices. I insisted test be re-taken to get 'correct' results. She also was UNABLE to find a blood vein for blood test withdrawal, having to 'poke' both arms several times with NO success. Blood needed to be withdrawn by 'Lab' person on-site at medical offices. These same 'difficulties' have occurred in the past at these medical offices by para-medical assistants..
