About Our Survey

The Patient Rating score is based on responses given during the CAHPS Patient Experience Survey. Responses are measured on a 10-point scale, with 10 being the best score. These scores are then translated to a 5-point scale in order to display results in a 5-star rating. Comments are also gathered from the same survey and displayed in their entirety with the exception of any language that may be considered slander, libel or contain private health information, which will be removed prior to publishing the comments.

Dr. Caraballo is an excellent and caring provider, looking out for you when you yourself aren't too concerned. Her caring concern for your overall health reflects her global knowledge, making you feel confident that regardless of what you personally think, she is going to make sure that you are indeed well so that there are no future problems. I trust her implicitly for this, as I have had other providers who simply look at you as a number and push you through the door, even when you are still not feeling like your usual self. I could ask for no better provider, nor would I want any other than Dr. Caraballo taking care of me and my health. Thank you for such a superior, caring individual who makes you feel comfortable and that your concerns were heard and attended to.

Providence Health & Services Patient

Despite running a little late (the office was busy during my last visit), I was very satisfied with the amount of time my primary care provider gave to me. She listened to my concerns and addressed the issues that worried me. She explained health concerns causes and treatments. I felt some relief in leaving that office.
