Paper medical records
Providence Health & Services provides access to medical records from our hospitals and other medical facilities in Oregon to patients and their authorized representatives. Get instructions for accessing your paper medical records from Providence.
Doctor's Office

While we are working to make Doctor's Office services available to all patients, not all clinics offer this service. Doctor’s Office services are currently only available at participating clinics and offered to patients 18 years of age or older, who have an established relationship with their Providence Medical Group primary care physician and have an active email account.

MyChart is a new service and is currently only available at PMG-Bethany. MyChart is offered to patients 18 years of age or older, who have an established relationship with their Providence Medical Group primary care physician. Ask your caregiver to turn on access to your records during an upcoming visit.
