If you plan to have surgery at any of our hospitals in Oregon, you can preregister online. Using online preregistration allows you to complete the forms in the comfort of your own home, reducing time and stress when you arrive for admission.
Get the best possible care.
To make sure you get the best, safest care possible, we need some important information about you. Before you come to the hospital for surgery, please complete and bring with you a copy of your Personal Medication History form and the Adult Health Status form. If the patient is a child aged 13 years or younger, the parent or guardian should complete the Pediatric Health Status instead of the health summary.
In many circumstances, your surgeon may want you to perform certain tests, visit the Pre-Operative Clinic or take/withhold certain medications before your surgery. Be sure to talk with your surgeon about your plan of care well in advance of your surgery date.
