Person-Centered Care and Support for Older Adults

Imagine a program that provides older adults with all their medical care, medications, and equipment and combines this with a network of support from social workers, therapists, and in-home caregivers. This program exists and it’s called PACE, a Program of All-Inclusive Care for the Elderly. PACE is both an insurance and a provider. Care can be at a Providence ElderPlace PACE health and social center, or at home when needed.

PACE is for adults 55 and older who need assistance with activities of daily living, such as using the toilet, mobility, cognition, or memory. By bundling medical care, senior services, and social activities into one program, PACE wraps your loved one in a network of support. This is the care older adults need, delivered where and when they need it.

What you pay depends on your financial situation. If you financially qualify for Medicaid services, you will not pay a monthly premium for PACE. Those who do not qualify financially for Medicaid pay the ElderPlace premium, which equals the current Medicaid rate. There are no co-payments or deductibles. ElderPlace covers all necessary medical and social services, and the premium rate does not change based on your changing needs.

PACE is authorized by the Centers for Medicare and Medicaid Services (CMS) and has certain eligibility requirements for enrollment. You can join PACE if you are:

  • Age 55 or older
  • Living within or willing to relocate to our service area (Multnomah, Clackamas, Washington, Clatsop, or Tillamook County).
  • In need of support services as defined by the state of Oregon. This generally means needing assistance with "activities of daily living" such as moving about (ambulating), memory (cognition), and/or using the bathroom (toileting)
  • Able to live in a community setting (includes family home or assisted care settings, and not a nursing facility)
  • Medicaid eligible or willing to pay private fees equal to the current Medicaid rate
  • Willing to receive all needed health care, including primary care and specialist physician services (other than emergency services) from the PACE organization or from a provider authorized by the PACE organization

Those enrolled in PACE are supported in their least restrictive environment (often their own home, an assisted living or residential care facility, or an adult care home).
