Overview

In-home services are one part of the comprehensive package you get with Providence ElderPlace, a Program of All-inclusive Care for the Elderly. The nationwide PACE model was founded on helping seniors continue to live in their own homes by offering varying levels of support, from housekeeping to meal preparation and more.

ElderPlace participants who are able to direct their own care and who desire to continue living in their own homes have access to the following services, in partnership with contracted in-home agencies:

  • Meal preparation
  • Shopping
  • Errands
  • Housekeeping
  • Medication reminders
  • Personal care, such as bathing, dressing and grooming

Our in-home service coordinator works closely with trained, professional caregivers to provide a seamless experience and to ensure quality care.

Along with their in-home support, ElderPlace participants also have regular access to our Health and Social Centers for medical care and social and therapeutic activities. Transportation to and from the Health and Social Centers, as well as all medical appointments and ElderPlace coordinated events, is provided by ElderPlace. The ElderPlace team also helps members transition to other levels of care, should the need arise.
