Our Health and Social Centers are the heart of the Providence ElderPlace community. This is where our members come together for the medical, social, recreational and therapeutic services that make ElderPlace stand apart from other programs for older adults. Located throughout Multnomah County and designated areas of Washington County, our centers maintain Providence's high standards of care while fostering independence for older adults.

ElderPlace Health and Social Centers: A combination of traditional and nontraditional

At each ElderPlace location, participants enjoy a number of scheduled activities throughout the day. Pet, art and music therapies are popular events, as is bingo. Members often gather to review books, discuss current events, share Bible studies, or knit and sew.

Participants enjoy a nutritious, tasty lunch during their visit. Registered dietitians oversee meals, and all special diets are accommodated.

Providence ElderPlace is an innovative program known as PACE (Program of All-inclusive Care for the Elderly), a nationwide, federally recognized program. They keep older adults as healthy as possible and living in the community for as long as possible.

What makes all PACE programs unique is that each social center has an attached medical clinic. So, while participants attend the social center for activities and meals, they also have access to their primary care team.  Geriatric-trained physicians and nurse practitioners, pharmacists, physical, speech and occupational therapists, case managers and more make up the primary care team at each center. Other primary care services available at the Health and Social Centers include dental care (by hygienists trained in geriatric care), audiology and optometry.

When a participant needs medical care beyond what is available at our on-site clinic, our ElderPlace team coordinates all referrals, appointments and transportation so that members can receive the care they need. Our ElderPlace physicians talk to the specialists treating their patients, thus providing a true “medical home” for each of our participants.
