Overview

Providence ElderPlace is a Program of All-inclusive Care for the Elderly (PACE) care model. ElderPlace coordinates all of the care services that seniors need to remain as active and independent as possible. ElderPlace is the only PACE program in Oregon and serves Multnomah County and designated areas of Washington County.

At the core of the PACE care model is the interdisciplinary team. This team is made up of doctors, nurses, therapists, pharmacists, social workers, caregiver aides, chaplains and others. Our primary care providers and other caregivers are trained in working with seniors and are focused on treating the whole person. They will work together, with each participant, to develop a care plan that is tailored to each participants needs.

The PACE care model covers all necessary medical care and supplies. There are no copays or deductibles. Because ElderPlace clinicians treat only ElderPlace participants, all of our members have a primary care provider and a primary care team and receive a lot of focused, individual attention. Supportive care from professionals such as our social workers and chaplains plays an important role in the ElderPlace care model. These professionals help participants address aging and loss issues and are integral to treating the whole person. Support extends beyond our participants to their families as well.
