Providence Cancer Survivor Program for Clinicians

Providence Cancer Survivor Program is open to patients who have completed treatment. We serve primarily patients who were diagnosed with breast, lung, and colon cancers, and lymphoma, but we have expanded to serve other patients as well. An advanced practice nurse offers a thorough evaluation, including history of diagnosis and treatment. The treatment summary and a personalized care plan are given to the patient and all involved providers. The care plan has recommendations for managing long-term side effects of treatment and a plan for surveillance. Our clinics offer cancer risk assessment and genetic counseling.
