Take charge of your health with InfoRx, a simple tool to help you learn about your health care options.
How does InfoRx work?
An InfoRx is an online educational tool prescribed by your health care provider. It is made up of medical facts and personal questions to help you participate in a choice about your health care.
Each of the options you're looking at tends to work equally well. However, each has its own risks and benefits. The best choice depends on what feels right to you after you've learned more.
InfoRx helps you make your decision.
How do I get started?
After your health care provider prescribes an InfoRx, you'll receive an email containing a link and an access code. If you don't have time to complete your InfoRx in one sitting, you can return later using the same link.
Once your InfoRx has been prescribed, you will have 60 days to complete it. Your InfoRx should take about 15 to 20 minutes to finish.
What happens after I complete my InfoRx?
Your results will be sent to your health care provider so that you can talk them over and decide on next steps. A copy may be saved in your medical record for future reference.
Please download a copy of your results before leaving your finished InfoRx. Your answers won't be saved on the InfoRx website, and you won't be able to use your decision aide again. If you'd like to see your answers again in the future, please ask your clinic for a copy from your records. Or, you can go over the facts and questions
again on your own.
Please contact your clinic if you have questions about your InfoRx or need to schedule a follow-up appointment.
