Providence Home Health Palliative Care
When you or a loved one is diagnosed with a life-altering or chronic illness, your life can change dramatically. You may need information and support to help you adjust to these changes. Providence Home Health Palliative Care can manage patient care and works to provide prevention and early intervention of issues that can lead to crisis and/or hospitalization.
Providence Home Health Palliative Care is an extension of our traditional Home Health program. Patients needing palliative care often have complex needs that require specialized care.
Providence Home Health Palliative Care is a part of our Home Services continuum of care, including Home Health, Hospice, Palliative Care and Connections.
Home Health Palliative Care offers:
- Management of difficult symptoms: Help to ease pain and breathing difficulties; reduce nausea and constipation; and address loss of appetite, fear, anxiety, confusion, depression and more.
- Help in addressing changing goals of care: The palliative care team helps identify the patient’s health care wishes; adjust care plans as symptoms change, or as requested by the patient; and transition the patient to hospice care, if necessary.
- Coordination of care throughout the illness: The palliative care team provides guidance regarding available treatment options; practical and emotional support for the family and caregivers; and counseling for individuals and families.
- Education: Knowledgeable team members help patients and family members or caregivers access community resources, support groups, patient education and information about specific diseases or conditions.
Referral Criteria: A physician’s order is required to obtain home health services. However, you may request information from Providence Home Health, and we will assist you with the process.
Payment: Medicare, Medicaid, worker’s compensation and most private insurance companies and HMOs cover the cost of home health care. Coverage and prior authorizations differ from plan to plan. Our staff will verify benefits and communicate with you about any financial responsibilities you may have.
Financial assistance may be available, based on income, if you do not have insurance. We know recovery and rehabilitation can be challenging to say the least. We at Providence Home Health want to help in practical ways to make you and your family’s experience a little easier.
