Providence Heart Clinic a bellwether for coordinated specialty care

Providence Heart Clinic at The Oregon Clinic Gateway
is one of 64 charter specialty medical homes nationally to be certified by the National Committee for Quality Assurance.
The recognition program is modeled after the NCQA's certification of primary care medical homes. Providence Heart Clinic is one of just five sites in Oregon to be certified, along with The Oregon Clinic, Portland Pain and Spine, the Immune Deficiency Clinic at Kaiser and Women’s Healthcare Associates.
The patient-centered specialty practices, as they are called, will be responsible for written referral agreements, delivering team-based care and medication management, among other standards. Read more about the NCQA Patient-Centered Specialty Practices Program.
