Chances are your child's heart is healthy – and yet a small percentage of young people have hidden heart defects that put their lives at risk, especially during physical exertion. For parents of children age 12 to 18, you may want to consider getting your child screened for free.
Play Smart screenings are free, but appointments are required. Appointments are scheduled up to a month in advance. What to know before you go
A parent or guardian must accompany the person who is to be screened. Screenings are open to young people ages 12-18. Comfortable gym or workout clothing is advised. Females youth are encouraged to wear a sports bra for the screening process. Participants are advised to abstain from sports or physical activity two hours before the screening.
Forms to complete
Please bring a signed consent form and a completed medical history questionnaire to the appointment or arrive 15 minutes early to complete the required forms. Paperwork must be completed in order for youth to be screened.
