Your doctor, your primary nurse and other members of the health care team will work with you and your family to plan your discharge. If you have questions about your post-hospital diet, activities or other matters, please be sure to ask.

Before you leave

Before you leave the hospital, please make sure you:

  • Have all your personal belongings
  • Understand all your discharge instructions
  • Have a location, time, telephone number and contact person concerning any community services you have been referred to
  • Know if you are participating in any follow-up care
  • Your nurse will assist you throughout the discharge process

Although our inpatient pharmacy does not fill prescriptions, you may wish to have your prescriptions filled at the Credena Health Pharmacy at PPMC Plaza. The pharmacy is located at 5050 NE Hoyt. As you prepare for discharge, please let your nurse know how you want your prescriptions handled. If you choose the Credena Health Pharmacy at PPMC Plaza, your prescriptions can be faxed to them ahead of time so they will be ready when you arrive.
