Home Services Southern Oregon

Providence is southern Oregon’s only provider of coordinated home health care. We can help you clarify your health care goals, explore options, and communicate with your doctor and your family. We even have an insurance specialist who can help you understand and navigate health insurance so you can get the most from your benefits.

Working under the direction of your doctor, our registered nurses, therapists and staff oversee your entire course of treatment. Home health services are provided in your home, whether that is a private residence, an assisted living community, an adult care home, or a residential or intermediate care facility.

Connections services are provided free of charge, thanks to support from Providence Community Health Foundation. To be eligible for services, a client must be diagnosed with a life-limiting illness and have a life expectancy of one year or less. Services are provided to people living at home or in any care facility.

Because you are a whole person, we strive to meet your physical, spiritual and social needs while adhering to your wishes and providing the highest quality care.

To request support, call one of our numbers listed below. A nurse care manager will talk with you and verify with your doctor which of our services is right for you.
