Providence ElderPlace Gresham

 

Participants of Providence ElderPlace in Gresham do not live at the site, but live in the surrounding community, either in their own home or with family, in an assisted living or residential care facility (ALF or RCF) or an adult care home (ACH). Participants visit the Health and Social Center anywhere from one to five times per week where they participate in activities, socialize with friends, join exercise groups and are seen by their primary care medical team. ElderPlace buses transport Participants to and from their home to the Gresham Health and Social Center, to medical appointments and to ElderPlace coordinated special events and outings. Participants who are homebound are served in their home setting.

After years of planning and 10 months of construction, Providence ElderPlace in Gresham opened its doors in August of 2003 and is currently our largest site.

The site features:

  • A large central area for activities, exercise groups or a space to spend time with friends
  • A horticulture room
  • A quite room for Participants who want to rest or require less activity
  • A community dining area with kitchen where meals are prepared on-site
  • Primary and specialty care medical exam rooms
  • Spacious exam rooms with exam equipment for larger adults
  • Large physical, occupational and speech therapy rooms
  • A garden area with wheelchair accessible pathways
  • Bathing rooms with wheelchair accessible showers

Providence ElderPlace in Gresham Participants are members of the Providence ElderPlace care program and receive all of the social and medical benefits offered by the PACE model of care, including:

  • All necessary primary and specialty medical care including dental, hearing, vision and foot care
  • All medically necessary equipment and supplies
  • Prescription and over-the-counter medications
  • Laboratory tests and procedures
  • Inpatient hospital care and outpatient surgery
  • Recreational and therapeutic activities
  • Transportation to off-site medical appointments and ElderPlace coordinated outings and events
  • Emergency services
  • Physical, occupational and speech therapy
